I guess that's the core of the debate. Observation alone is not scientific method, though it falls into "Science" as a field today. You need to be able to take action for there to be an experiment and validate / falsify it. Otherwise, it's just an opinion.

If we don't know it's harmful then there is no ethical problem asking people to volunteer. It can be an experiment according to the scientific method. (FWIW, it's why animals are used in the first forms of testing; still sucks but humans rationalize it that way.)

The way they try to do that is by using known biomarkers as a proxy for health. Then you can setup trials to monitor the effect of treatment/drug/habbit by looking at those markers. It's scientific within the assumption that the markers are connected to life expentency.

I guess I'm saying that you can only do science on things you can act upon (back to the RL metaphor). Science to create a world model only is not the scientific method, it's just observation / speculation / hallucination that can't be falsified.
